Sub: Inclusion of name under Swasthya Sathi Scheme of Govt. of West Bengal
Dear Sirs,
We are pleased to inform you that at the behest of Ms. Mamata Banerjee, the Hon’ble Chief Minister Govt. of West Bengal, all former Sports persons who have participated in International, National and State Level Sports Events (which includes Badminton) are now entitled to be included in the Govt. of West Bengal “Swasthya Sathi Schemeâ€. Application in prescribed Proforma must be submitted with credentials within April 2018.
The Application Form with its Annexures are enclosed.
Duly filled up Application Forms by the eligible persons must be submitted within the month of April, 2018 directly through Email “ health.sports.wb@gmail.com†or be deposited at “Dept. Of Sports, New Secretariat Building, 6th floor, 1, Kiran Sankar Roy Road, Kolkata – 700001†or at the office of Youth Officers in the Districts.
Other than Copies of the Applications, WBBA shall not acknowledge receipt of any original, which please note.
You are requested to forthwith circulate it to all former eligible Players of your District immediately.
